Howe Family Dentistry accepts most major dental insurance plans. We are a preferred “in-network” provider for:

We also welcome patients who do not have dental insurance and/or prefer to pay for their dental care directly. Whether you have dental insurance or pay directly, we are committed to the same level of excellence in the care we provide.
The following is our Statement of Financial Policy. When you come to our office, we will give you a copy to review and sign prior to treatment in order to avoid any confusion on how we collect payment for any dental fees your incur under our care.
All patients prior to seeing the dentist must complete our Health History and Insurance Form.
Our customary fees are the same for all patients whether or not they are insured. Each patient is financially responsible for his or her own account. As a courtesy to those patients with insurance our office will be happy to file an electronic claim on your behalf. Some insurance policies may allow payments based on a fixed fee schedule which may differ from our usual fees, we ask that you are familiar with your policy benefits and limitations. Please be aware that insurance may vary greatly in types of coverage they offer. If within 60 days we have not received payment from your dental insurance we will request that the balance be paid in full. For those patients with dual insurance we will only accept assignments of benefits from the primary plan. All balance after payment from the insurance company will remain the patient’s responsibility.
As an added service we will provide an itemized treatment plan for any patient needing additional dental treatment. It will list all services needed, an estimation of insurance benefit and an estimated patient portion.
In keeping with the highest level of professional care and maintaining our fees we have established the following payment options for our patients:
- Payment is due at the time of service. Cash, check, Visa, MasterCard, and Discover are accepted payments.
- Insurance, the estimated patient portion is due at the time of service. After payment from your insurance company or should we not receive payment within 60 days, the full balance is due according to financial arrangements made in advance. Should there be any difference in the insurance company’s determination of usual and customary rates, it will be the responsibility of the patient to pay the remaining balance.
- We offer CareCredit, a dental financial plan, which allows qualified patients to make payments over a period of time. You may apply online or in our office for CareCredit. Please feel free to ask any questions you may have.
